Your medical records are available to you free of charge from the Patient Portal. To login or register for a Patient Portal account, please follow this link or use the button below.

If you prefer not to obtain your records through the Patient Portal, you can print and fill out one of the following forms and, for a fee, we will deliver hard copies of your medical records to you.

Completed medical records release forms can be dropped off in person at any of our locations, or faxed to our Medical Records Department at (757) 961-2971.
